COORDINATED CARE
Patients are referred to our cardiopulmonary rehab specialist by their primary care provider. Medical records and goals are evaluated by the specialist who will schedule necessary tests. Through supervised testing, exercise, emotional support, and education quality of life can be restored and maintained. Graduates from our program report improved endurance and appreciate personalized health plan.

Criteria for admission to Cardiopulmonary Rehab:
Cardiac

- Heart attack in the last 12 months
- Coronary artery bypass surgery
- Current stable angina (chest pain)
- Heart valve repair or replacement
- Coronary angioplasty (stent)
- Heart or heart-lung transplant
- Stable chronic heart failure
Pulmonary (including respiratory services)

- Emphysema
- Interstitial Lung Disease
- Symptoms of COVID-19
