Medical Records
Medical records are confidential documents kept for the mutual benefit of the patient, provider and other health care professionals. They are the property of Jefferson Healthcare Hospital but, under most circumstances, the patient has a right to the information. As part of the admission process each patient routinely signs a Consent for Release of Information. This consent releases information only to health care providers and employees who have a “need to know” for purposes of the patient’s medical care or to secure payment by designated responsible insurers.
Medical Records Office
For your convenience, the medical records request form is available online and can be downloaded here. You can also pick up and drop off a form at the reception desk and at the financial services office off the main lobby of Jefferson Healthcare.
Jefferson Healthcare’s Medical Records (HIM) office processes all requests in the order that they are received. Their goal is to process each request as quickly as possible and as required by Washington State law which is within 15 days of receiving the request.

The HIPAA Privacy Rule gives individuals a fundamental new right to be informed of the privacy practices of their health plans and of most of their health care providers, as well as to be informed of their privacy rights with respect to their personal health information.
