How the program works
The diabetes educator and dietitian meet with patients to determine the level of diabetes care required. Appointments focus on patient progress and modifications to achieve set goals.
For individuals with “pre-diabetes” or high risk for Type 2 diabetes. Learn the lifestyle recommendations to prevent Type 2 diabetes. Diabetes Prevention – Getting Started, a 2 hour class for the prevention of Type 2 diabetes, is offered every 3 months.
Most insurances, including Medicare and DSHS, include a Diabetes Education benefit. Please contact your plan for coverage details.
Free Monthly Support Group: Meetings are held the third Tuesday of each month from 12 – 1:30 pm. Location: Avamere at Port Townsend Recreation Room, 1201 Hancock St. in Port Townsend (Formally Seaport Landing).
Living Well with Diabetes is a class series for newly diagnosed individuals or for those needing a refresher in diabetic care. Each series meets on Mondays at the Jefferson Healthcare Wellness Center from 10 am to 12:30 pm. See the Events schedule for class series dates.
Diabetes Drop-in Clinic, once a week on Thursdays from 10-12 pm, is available for quick blood sugar checks, meter checks and basic questions.
Dexcom and Freestyle Libre Continuous Gluocose Monitors are available and covered by most insurance if you use insulin.

Diabetes self-management education at Jefferson Healthcare is recognized by the ADA as meeting the national standards for diabetes self-management education. To maintain recognition status, ADA guidelines and standards are continually followed. This process helps to ensure the quality of the services provided to the community we serve.
