Diabetes Care

A program for self-care

Our diabetes self-management education is based on individual needs to empower those with diabetes to live healthier lives.  Our healthcare professionals work with patients to offer a range of services and education options to meet the needs of patients with pre-diabetes.

How the program works

The diabetes educator and dietitian meet with patients to determine the level of diabetes care required. Appointments focus on patient progress and modifications to achieve set goals.

Diabetes Prevention

For individuals with “pre-diabetes” or high risk for Type 2 diabetes. Learn the lifestyle recommendations to prevent Type 2 diabetes. A 2 hour class for the prevention of Type 2 diabetes is offered every 3 months.
