A Program for Self-Care

Our diabetes self-management education is based on individual learning needs to empower those with diabetes to live healthier lives.  Our healthcare professionals offer a range of services and education options to meet the needs of people living with diabetes.

INDIVIDUAL EDUCATION APPOINTMENTS

Education and support on a range of diabetes self-management topics.  Including training for those starting insulin therapy, insulin pump therapy or using a continuous glucose monitor.   Education is based on participant needs and the provider referral orders.

LIVING WELL WITH DIABETES group Class series

4-part class series for anyone with a new diagnosis of diabetes or for those who have not previously participated in diabetes education classes. A provider referral is needed, and the class is covered by most insurance.

TYPE 2 DIABETES PREVENTION Virtual Class

For individuals with “pre-diabetes” or high risk for Type 2 diabetes. Learn lifestyle recommendations to prevent Type 2 diabetes. This virtual class is offered every other month.

ONGOING SUPPORT

We offer a monthly virtual meeting, with a variety of topics and speakers. All are welcome, and there is no fee. Meetings are held the 3rd Thursday of each month, 12:00 – 12:30 pm via Zoom. To be added to the email list please call 360-385-2200 ext. 1240

Insurance Coverage

Most insurances, including Medicare and DSHS, include a Diabetes Education benefit. Please contact your plan for coverage details.

Certificate of Recognition by the American Diabetes Association (ADA)

Diabetes self-management education at Jefferson Healthcare is recognized by the ADA as meeting the national standards for diabetes self-management education. To maintain recognition status, ADA guidelines and standards are continually followed. This process helps to ensure the quality of services provided to the community we serve.
