A program for self-care

Our diabetes self-management education is based on individual needs to empower those with diabetes to live healthier lives.  Our healthcare professionals work with patients to offer a range of services and education options to meet the needs of patients with pre-diabetes.

Steps to ensure your safety

Our COVID-19 response ensures that every person entering the hospital and clinics is screened for temperature and respiratory illness symptoms.  Universal masking is in place for all patients, visitors, and employees.  Exam rooms and waiting areas receive extra cleanings and we have created physical distancing between patients.

Type 2 Diabetes Prevention

For individuals with “pre-diabetes” or high risk for Type 2 diabetes. Learn the lifestyle recommendations to prevent Type 2 diabetes.  Diabetes Prevention – Getting Started, a  2 hour class for the prevention of Type 2 diabetes, is offered every 3 months.

Living Well with Diabetes

4-part class series for a new diagnosis of diabetes or for anyone needing a review lifestyle management.  Provider referral is needed and the class is covered by most insurance.

Ongoing Support

Free Monthly Support Group: Meetings are held the third Tuesday of each month from  12 – 1:30 pm. Location: Avamere at Port Townsend Recreation Room, 1201 Hancock St. in Port Townsend (Formally Seaport Landing).


Diabetes Drop-in Clinic, once a week on Thursdays from 10-12 pm, is available for quick blood sugar checks, meter checks and basic questions.

Individual Education Appointments

Education and support on a reange of diabetes self-managment topics.  Including trainig for those starting insulin therapy, insulin pump therapy or usinga continuous glucose monitor.  Also instruction in problem-solving skills to optimize treatment options.  Education is based on participant needs and the provider referral orders.

Insurance Coverage

Most insurances, including Medicare and DSHS, include a Diabetes Education benefit. Please contact your plan for coverage details.

Support Groups

We offer a monthly meeting, with a variety of topics and speakers. All are welcome, and there is no fee. Meetings are held the 3rd Tuesday of each month, 12:00 – 1:00 pm at Avemere at Port Townsend Recreation Room, 1201 Hancock St. in Port Townsend.  (Formally Seaport Landing)

Certificate of Recognition by the American Diabetes Association (ADA)

Diabetes self-management education at Jefferson Healthcare is recognized by the ADA as meeting the national standards for diabetes self-management education. To maintain recognition status, ADA guidelines and standards are continually followed. This process helps to ensure the quality of the services provided to the community we serve.
