Transgender patient care navigator support

Our primary care providers understand the importance of receiving gender-affirming care close to home.  We partner with you and regional consultants to provide the best care options to meet your health goals and needs.

 

If you are seeking a primary care provider who best meets your needs, please contact our Transgender Navigator, Jackie Levin, RN, at 360.385.2200 ext. 2235. Our transgender navigator can also help you find local mental health and community resources for connection and services such as gender-affirming surgeons.

 

If you have any patient care concerns, please let her know, so these can be addressed.

Whole Person Primary Care

Our primary care teams provide preventative health care and ongoing assistance in the diagnosis and management of new and chronic medical issues.

 

Many of our primary care providers have experience and education specific to care for transgender and gender-diverse patients.  They know how to work with your specialists to ensure the best possible care and coordination of services.

Individualized Care

LGBTQIA+ and gender-diverse individuals have routine healthcare needs and may also be at increased risk for certain medical and behavioral health conditions.  Our providers are dedicated to caring for you as an individual.  Behavioral Health services are integrated with primary care teams and work in collaboration with your primary care provider.  Providers utilize evidence-based techniques to treat common conditions such as anxiety, depression, PTSD, attention deficit disorder, and substance abuse disorder.

 

Many transgender and gender-diverse patients are interested in hormone therapy. Our providers will work with you as an individual to provide the best care options available for hormone treatment. This approach sees gender affirmation as a routine part of your primary care, though it may require a referral to a specialist to initiate hormone therapy.

Preventative care

Critical to maintaining your health, preventative care may include immunizations, vaccines, screenings, and regular checkups.

Hormone Replacement Therapy

Our providers can coordinate hormone replacement treatment.

Behavioral Health

Behavioral health services and therapists are available through provider referral.

Additional Services

For services not currently provided at Jefferson Healthcare, such as gender-affirming surgeries, referrals are provided.
