Reactivate your Teen MyChart Account for Greater Access to Health Information.

July 1, 2026

Starting June 30, patients aged 12-17 will use a new Teen MyChart account to improve access to their health information. Existing MyChart accounts for this age group will be deactivated and need to be reactivated.

Deactivation: Existing MyChart accounts for this age group will be temporarily deactivated and need to be reactivated.

Reactivate or Sign Up: To reactivate your Teen MyChart account or sign up for a new one, let our staff know during your next appointment. We will verify your identity and help you set it up. If you prefer not to wait, you can also visit any of our clinic locations after June 30 for assistance.

When teens reactivate their accounts, they can:

  • Manage appointments
  • Request prescription renewals online
  • Communicate with healthcare providers
  • Review health info and instructions
  • Get test results online
  • Check their medical history, medications, immunizations, and allergies

Confidential Health Services

It’s important for teens to know that their new MyChart accounts may include private information. Jefferson Healthcare encourages involving parents or guardians in healthcare decisions when possible. If needed, staff can help with discussions between teens and their parents or guardians. However, minors can receive certain healthcare services confidentially without parental consent.

Proxy Access

Proxy Access allows parents or guardians to stay involved without needing the teen’s login. This change will not affect proxy access.
