Jefferson Healthcare achieves accreditation with ACHC

Jefferson Healthcare is proud to announce it has achieved accreditation through Accreditation Commission for Health Care (ACHC) for Hospice and Palliative Care.

Accreditation is a process through which healthcare organizations demonstrate compliance with national standards.  Accreditation by ACHC reflects an organizations dedication and commitment to meeting standards that facilitate a higher level of performance and patient care.

ACHA is a nonprofit organization that has stood as a symbol of quality and excellence since 1986.  ACHA is ISO 9001”2015 certified and has CMS deeming authority for Home Health, Hospice and DMEPOS.

The Jefferson Healthcare Hospice Care and Palliative Care teams consists of a medical director, hospice nurses, social workers, home health aides, clergy, therapists, and trained volunteers who provide each client with holistic care to make each day for the client and family as meaningful as possible.   Each client is cared for by an individually assigned nurse who makes both regularly scheduled and as-needed visits to manage pain and symptoms.  The goal is to help clients be comfortable while also allowing them to remain in control of their lives and sufficiently alert to be able to make important decisions.
