COVID-19 booster appointments for children 5-11 years old

May 20, 2022 – With the approval from the Advisory Committee on Immunization Practices’ (ACIP) the CDC now recommends COVID 19 vaccine booster doses for children between the ages of 5 and 11 years old.  Jefferson Healthcare Sheridan clinic is now scheduling booster appointments for children 5-11 years old, who have already received two doses of the vaccine (with the 2nd dose being at least 5 months ago).   Appointments can be scheduled by calling or texting 360.379.8031.

Children are required to be accompanied by their legal guardian or have the original Patient Acknowledgement form signed by a legal guardian.  No photographs, copies, or over-the-phone verbal consent can be accepted.  Please download and complete acknowledgment forms from www.jeffersonhealthcare.org/covid-19-vaccine/ and bring previous vaccination card.
